About the role

Your role as a Social Worker within the Annual Review Team will include the following:

  • To have a passion for working with an interesting and diverse group of Adults who may be Older, or have Physical or Sensory Disabilities
  • To review the care and support already in place, and develop an appropriate ongoing care plan with the service users, in line with Care Act 2014 eligibility criteria
  • You will be confident in applying the Strengths Based Approach to promote the service users independence and community engagement
  • To provide supervision, and uphold and improve the standards of practice within the service

About you

We are looking for candidates who are passionate, supportive, motivated and professional in their conduct. We are looking for candidates with knowledge and experience of the following:

  • You will be a registered Social Worker having completed your ASYE
  • You will have knowledge of and experience in applying the Care Act, Mental Capacity Act and Mental Health Act
  • You will have an ability to communicate effectively with a range of other professionals, service users and carers
  • You will have the ability to record accurately and produce reports in a timely manner
  • You will have the ability to organise your own time and work quickly
  • You will be able to support, manage and organise others.

To be successful in the role you will need to demonstrate the following:

  • Initiative:The ability to spot and seize opportunities and to act without being told
  • Flexibility/Adaptability:Ability to adapt readily to change and work effectively in a variety of situations, groups or with individuals
  • Managing Oneself:The ability to achieve performance through personal organisation, showing confidence in one’s skills, capability, judgement and self control in stressful situations
  • Influencing: The ability to persuade others to a particular course of action or viewpoint whilst respecting their concerns, motives or feelings
  • Organisational Awareness:Understanding who are the key people to influence/interact, both within and outside the County Council and building good relationships with them
  • Communication Skills:The ability to receive, understand and convey information and ideas effectively, using skills in oral and written communication and presentation
  • Decision Making:The ability to make decisions that optimise results.

Other information

Access to / use of a vehicle is an essential requirement. Flexible working is available to staff following a successful probationary period.

This post is exempt under the Rehabilitation of Offenders Act 1974. Due to the sensitive nature of the duties the post holder will be expected to undertake a criminal record check as part of the recruitment process.

Induction: Our induction takes place over 6 weeks and includes shadowing, e-learning, training courses, meeting with other relevant health professionals and visiting day services.

About the Business Unit

The Annual Review team is a large and supportive team made up of Social Workers and experienced Social Work Assistants. The Annual Review Team covers the entire county of Buckinghamshire and, as such, we can usually accommodate workers who wish to work in a specific geographical area. We work directly with people who may be an Older Adult, or an Adult with Physical or Sensory Disabilities and work both in the communities and in residential care facilities.
